Great topic! I think that this is one of the hardest lessons that we have to learn in life.
Being responsible does not always mean that you are to blame. Blame indicates that there is an intentional wrong doing.
Sometimes things just happen which we didn't see coming. We can take responsibility for the outcome knowing that we are not to blame. I think that this small difference helps leaders to step up and shoulder the responsibility.
